Individual and Family Plans

SelectHealth's Individual plans are designed for individuals and families without employer-sponsored coverage or those who are looking for a better coverage solution. We provide both standard HMO/Plus products as well as High Deductible Health Plan (HDHP) which is designed to be used with a Health Savings Account (HSA). Both plan designs are offered in conjunction with our three comprehensive provider networks. Individual plus plans offer a point-of-service or 'plus' feature. This means you can choose both participating and nonparticipating providers.

Select Value offers access to 19 participating hospitals and 1,100+ participating physicians in Davis, Salt Lake, Weber, and parts of Summit and Utah counties.

Select Med Plus®

Select Med Plus offers access to 28 participating hospitals and 3,000+ participating physicians in Cache, Davis, Duchesne, Iron, Juab, Millard, Morgan, Salt Lake, Sanpete, Sevier, Summit, Utah, Wasatch, Washington, Weber, and parts of Box Elder, Garfield, Piute, Tooele, Uintah, and Wayne counties.

Select Care Plus

Select Care Plus offers access to 34 participating hospitals and 3,700+ participating physicians in Beaver, Cache, Davis, Duchesne, Iron, Juab, Kane, Millard, Morgan, Piute, Rich, Salt Lake, Sanpete, Sevier, Summit, Uintah, Utah, Wasatch, Washington, Wayne, Weber, and parts of Box Elder and Tooele.
