SelectHealth Kids Plans
SelectHealth Kids plans are tailored to meet the needs of children of all ages. Covered benefits include well-child exams, immunizations, prescriptions, ER visits, and more. SelectHealth Kids plans are designed to provide your child with comprehensive coverage they need at a price more affordable than other insurance alternatives. Kids Plus plans offer a point-of-service or 'plus' feature. This means you can choose both participating and nonparticipating providers.
Select Value® Kids
Select Value Kids offers access to 19 participating hospitals and 1,000+ participating physicians in Davis, Salt Lake, Weber, and parts of Summit and Utah counties.
Select Med Plus® Kids
Select Med Plus® Kids offers access to 28 participating hospitals and 2,700+ participating physicians in Cache, Davis, Duchesne, Iron, Juab, Millard, Morgan, Salt Lake, Sanpete, Sevier, Summit, Utah, Wasatch, Washington, Weber, and parts of Box Elder, Garfield, Piute, Tooele, Uintah, and Wayne counties.
Premium Assistance Program
Families enrolled in SelectHealth Kids Plan who qualify for the Premium Assistance Program may receive up to 60 percent of their premium paid, depending on their annual household taxable income.
Apply for a Kids Plan
To apply for a SelectHealth Kids Plan, compete either the print version or form-fill application and sent it to SelectHealth. If you have an insurance agent, be sure to enter their name on page 5, section L*.
